PERMISSION TO RUN

A reminder to all Players looking to go out on the floor with any JR teams.

All players that are interested in trying out for a Jr. club MUST follow the following steps in order to do so:
- You MUST register with your home Centre (CMLA)
- Fill in the form here.
- A seperate form will be needed for each Jr Team required.
- The Club Registrar and Club President will asses each request
- Please allow for a minimum 48 hour turn around
- take letter with you for tryout
